Title: Clusters at night and migraines during the day?
Post by bigmenawa on Mar 19th, 2009 at 11:24pm
ok, i have posted on here that i was diagnosed last september with CH.  this is my second cycle and it started january 29.  i normally have 2 CH hits per night between 2 and 3:45 a.m.  i have had the normal migraines since i was 9 but i normally don't have them during a cycle.  for the past two days, including today, i have had a terrible migraine which made me throwing up sick.  i am taking topamax and indocin for the CH's.  i am new to this CH beast since this is only my second cycle so i guess what i want to know is this:  is it normal to have migraines the day after night hits?  i didn't have any migraines the first cycle .  what could be causing them now?  i appreciate all the valuable information i get from you guys who have suffered from CH so long.  you are really a blessing so thank you so much.  i love this forum.

Well, I've got both Migraines and CH, I've had hits at the same time as the migraines. So it is possible. However, I can't answer if that's why you're having them like this...

Is there anything that you've done differently this cycle, that you weren't doing the last one? Anything new that you're eating, or being exposed to?

eating habits are the same, exercise regimen the same, sleep cycle is terrible this cycle though.  it is springtime here in alabama and there is lot of pollen already.  these are not sinus-allergy type headaches, just good ole fashioned migraines.  i've had one for the past two days.  both started around midday and lasted for about 6 hours.  both days i threw up.  i toll midrin and tramadol which helped a little.  i can't think of anything i'm doing that's different than the first cycle.  i know i have been getting nailed at night and now sickening migraines during the day.

Thank goodness I didn't catch the double curse of CH and migrains. I haven't ever seen any of the "dual diagnosis" people post they thought one med was triggering the other. Just crappy coincidence I guess. :-/ Hang in there.

I am going to have to paraphrase from my neurology consultant.

The thinking is that whilst the mechanism in the brain is switched on for CH [activates in a different part of the brain] the part that causes migraines is switched off. Both CH and migraine involves the TMN *ouch*
For a person with these two primary headache disorders they can have migraines that can mimic some of the stabbing and boring of CH during a migraine but its not a true CH as it would be in a CH cycle bout.
When in a CH bout the cluster headaches are true cluster headaches but can also mimic some of the symptoms seen with migraine like throwing up.
This is why so many GPs tend to get into such a mess when faced with a person who has both migraine and CH. They expect the headaches to fit nicely into boxes and they refuse to.
With me so far.
If your lucky you have episodic CH that gives you a few weeks of hell then a few weeks of migraine and back again. If your unlucky you have a few days or the wretched thing may switch daily [definatly one for a consultant neurologist headache specialist and not any old neurologist]
IF your being horribly sick you may need an anti-emetic it can help a lot and stop you getting dehydrated which usually makes the headaches worse.
It may also be worth considering sumatriptian or similar if you can take triptains with the CH as I found  that could often last over 24 hours and soothe any after shock 'migraine like tremors' that went with bad CH headaches.
It might be worth looking at dose of your topamax with your doc, it tends to need tweaking as you move between CH and migraine to nail both of the little swines to the floor.

Hope this settles down for you really soon.

Dear Randall
its same for me also. I am episodic, my episode started from one month ageo,I took prednisolone under supervision of a neuro. now I have cut the prednisolone and hope to cut the cH.
at the morning i had migrane and now one hour ago i had a CH attack.
so its may be same as you.
however i take ergotamine in both cases , and in migrane some times i take Excedrine .
this is our life, challenging with headache.
wish you painfree times

thank you farzad for your post and encouragement.  i had to go today and have 2 different mri's, one for the brain and the other for the neck area. i also had a mra which shows the arteries and veins in your head.  i was in the mri tunnel for an hour and 3 minutes.  the mra was a twenty minute scan divided into two parts.  the first 14 minutes was a normal scan and then i was given an injection of some type dye.  the last 6 minutes showed the vessels with the dye in them.  i haven't gotten the results yet.  maybe on monday.  the nightly CH's and then the daytime migraines are getting old very fast.  we will just hang in there and beat this thing.  thank you and i hope you have a lot more PFD's.  thanks for responding.

Glad to hear I'm not the only one.  I was told this weekend that at the ER that it looks like I have both as well.  I hope to have that ruled out by the neurologist soon, I don't want to have  both at once...LOL

well i never took any of the fancy drugs you all use but last cycle i finished off with some good wopping migraines during the day. the ch hits at night were enough to wake me up but controllable with o2. at the very end i finished off with just migraines. do you think it's possible that your cycle is nearing the end?.....hopefully ;)

I've had both at the same time, and it is hell.

If I have a migraine, when a CH hits me iit's without warning because I didn't feel the shadow come on.

Dancing with a puke bucket is not fun.

i almost always have a constant headache during the demon days, maybe not migraines but enough to keeo me from daily routine, have to wear sunglasses, working out is out and loud noises kill.  i thought it was part otf the deal, a package.

Oh heck - it's just a headache - what are ya'll bitching about?  :-? :-?

Just kidding - put down the bats... ;)  I'm a double-timer also... And it's no fun when they  both hit at the same time. Can't lay down with the CH and can't sit up with the migraine - Luckily I don't get them both at the same time often, but when I do - no human or animal wants to me anywhere near me.

Don't know what to tell you -- just hang in there -- Someday someone is going to find a cure -- sigh!!!

Hugs BD :-*

I had that at its worst while on Topamax.  I wonder if there is some sort of link?

Anyway, I've said it before and I'll say it again, get a TSH thyroid test.

There was a study done recently that correlated new persistent daily headache with low thyroid. That constant headache feels just like a migraine to me.

Ask what the number is, don't just take "it's normal" for an answer b/c the guidelines have changed and many doctors aren't aware. Old guideline was anything above 5 required thyroid treatment, new guideline is anything 3+ (or even slightly lower if you have other symptoms) requires treatment, which is a $4/month generic at most major pharmacy chains these days.  The thyroid pills got rid of my migraines, helped me sleep through the night better than topamax, and made my CH WAAAAAYYYYY more manageable.
