We've put together a short survey on cluster headache conferences.
Whether you've been to any conference or not, your thoughts and opinions would be greatly appreciated and will help with future planning.

The survey goes offline while I was correcting the errors.   OK, then I forgot to open it again for a bit.

It seems to be working now, from IE or FireFox.
Clear your cache/history files and try again.

At the bottom of the page is a link to the survey.
As that link is in .html format it may work for you.
If not, I don't know what to suggest.
