Hoping your all doing ok. I've been having a couple of twinges over the past couple of weeks and since the last attack started late Sep last year got a bit nervous. Been and seen the GP and shes stocked me up on sumatriptan and vemaprill, and aggreed to order o2. Problem is the receptionist is the one who orders the O2 and she cant actually order anything correctly. First a tiny tank at 2lpm. Then came a large tank at 15lmp but a rebreathing mask that supplys 20% O2. Does anyone have any experience in filling out the HOOF form so I can do it for her?

I've attached a copy of the HOOF filled out for cluster headache sufferers.  I'm told this version works just fine in obtaining the right equipment at 100% oxygen concentration.
