Is it safe?
Dec 31st, 2008 at 10:05am

Hi, it is great to see this board is here! I came because I have a specific question, but I guess it is appropriate to start with background.

I am a 54 year old female. I started with clusters in my early 20s and for a couple of decades had very difficult periods: two per year of around three months with multiple, long-lasting headaches every 24 hours.

But I am happy to report that the severity of my problem has gradually reduced, especuially in the last few years. Now, in most years, I get only one cluster period in the late fall/early winter. I mainly have been getting off lightly with three week periods, one headache per day, usually at night. I thank God for the change.

I cope with the pain in a variety of ways. Deep, fast breathing and rocking in bed seem to be what I have settled on lately. I have not taken meds for the past several years, since they never did much and the periods are not as bad.

But the headaches still are what they are.

I have been in a cluster period for 2 1/2 weeks that may be finishing (I hope, I hope). I did not get my 2:00 a.m. headache last night. And the night before, the headache was shorter than usual.

Anyway, to my question. When not in clusters, I am a light, social drinker. Usually I wait a week after a cluster period to take even a sip of alcohol. But it is New Year's Eve, and I am going to a formal event. I also am invited to a party over the weekend.

It will have been about 48 hours clear when the New Year comes in. In my circumstances, would any of you chance a sip of champagne tonight? If not tonight, this weekend?

Thank you very much for your help, and my sympathy to those still having more severe cluster periods.

Individual variation is a big deal with CH and there is no "rule" on when to use alcohol. You have to try and see but there is less chance of an attack if you stick to white wine for a few more days. It's less a trigger compared to distilled spirits and red wine.

Given your short active periods, you might consider asking your doc for a sample of this med. A number of us have had excellent reponse, it's rapid acting, the ease of a pill, and low per dose cost. If it works you could then get an Rx from you doc for a small quantity to meet the needs of your brief cycle.

Olanzapine as an Abortive Agent for Cluster Headache.

Rozen TD.

Department of Neurology, Jefferson Headache Center/Thomas Jefferson University Hospital, Philadelphia, Pa.

OBJECTIVE: To evaluate olanzapine as a cluster headache abortive agent in an open-label trial. BACKGROUND: Cluster headache is the most painful headache syndrome known. There are very few recognized abortive therapies for cluster headache and fewer for patients who have contraindications to vasoconstrictive drugs. METHODS: Olanzapine was given as an abortive agent to five patients with cluster headache in an open-label trial. THE INITIAL OLANZAPINE DOSE WAS 5 MG, AND THE DOSE WAS INCREASED TO 10 MG IF THERE WAS NO PAIN RELIEF. THE DOSAGE WAS DECREASED TO 2.5 MG IF THE 5-MG DOSE WAS EFFECTIVE BUT CAUSED ADVERSE EFFECTS. To be included in the study, each patient had to treat at least two attacks with either an effective dose or the highest tolerated dose. RESULTS: Five patients completed the investigation (four men, one woman; four with chronic cluster, one with episodic cluster). Olanzapine reduced cluster pain by at least 80% in four of five patients, and two patients became headache-free after taking the drug. Olanzapine typically alleviated pain within 20 minutes after oral dosing and treatment response was consistent across multiple treated attacks. The only adverse event was sleepiness. CONCLUSIONS: Olanzapine appears to be a good abortive agent for cluster headache. IT ALLEVIATES PAIN QUICKLY AND HAS A CONSISTENT RESPONSE ACROSS MULTIPLE TREATED ATTACKS. IT APPEARS TO WORK IN BOTH EPISODIC AND CHRONIC CLUSTER HEADACHE.

--------------------------------------------------------------------------------

Olanzapine has a brand name of "Zyprexa" and is a antipsychotic. Don't be put off by this primary usage. Several of the drugs used to treat CH are cross over applications, that is, drugs approved by the FDA for one purpose which are found to be effective with unrelated conditions--BJ.

Welcome to CH.com, Diana.  I'm happy to hear that your cycles have been getting less frequent and milder.  Although mine have not gotten milder, they have certainly become less frequent.  There has to be some compensation for this aging business, eh?

As for the alcohol on New Year's Eve...just me, but I wouldn't chance it.

I normally wait seven days without a hit or a shadow before trying alcohol, and I'm a thrill-seeker.   Wink  Seems like a lot of people here wait a month or more.

Best wishes,

George

I wait at least two weeks.   Only then do I feel "safe."

What ever you chose to do... I hope  you have a good outcome.   Enjoy your evening!

You just have to ask yourself if it's worth it to risk an attack just for a sip of booze...I'm no teetotaller, but if there's a chance that it will bring on a cluster, why risk it for one sip of bubbly?  bring a bottle of sparkling grape juice instead- and make a toast to being pain-free!!  Wink

Thank you for all the welcomes and the advice. I guess part of me knew the prudent thing, but I would have been tempted if not for your responses.

Anyway, I avoided the New Year's drink and am still pain free as of this morning. That sure is worth a lot.

Thank you again!
