New Guy...With Questions...

Hello to all fellow suffers.  I have been suffering for about a week.  Long history of headaches but have not had severe CHA in about 5 years.  I was never officially diagnosed with CH due to other headaches complicating the picture (even had a pointless surgery to remove a mass in my forehead that might have been the source of the pain)...but now I have CH...no doubt.  I want to write more about my history but for now I have some questions it that's OK...

1. I have been given an 8 day taper of predisone...I have taken it before (including injecting myself daily for a month) and I never got any results but I just began a CH cycle so is it worth trying?

2. I have 6 mg vials of injectable Sumatriptan...I am afraid it will actually prolong the cycle...and, should I inject 2 mg if I use it?  I only have 5 vials and the RX says I can only use it 2 days a week...what good is 2 a week...I need to use it 2 times a day!

3.  I have O2 tanks (E 682 liters) but I am going through them a tank a day!  They only last for two - twenty minute sessons at 15 lpm...Maybe if it helps I should get the mask that stops the flow when you're not inhailing?

4. Should I use the O2 and the Imitrex at the same time?  I did get some relief from the O2.

5. This is important....I have been reading this site and I don't see anyone like me...namely, when I finally get the INTENSE pain directly over my right eye to clam down I still have a terrible general headache all over my head and the site of intense pain remains sore all day.  Does anyone else get a 'global' headache following a bout of CH pain?

Thanks for any advice on my questions.  I have tried Indomethicin (Indocin) and I just don't know if it helps...I also have injectable Toradol.

By the way...my particular CH come on after I fall asleep...about 45 min. to 1 hours into sleep....BAMM!  Screaming pain over my right eye...I have had 3 large kidney stones and this CH pain makes that look like nothing!

Thanks,  lastly...why is it that if this pain syndrome is so severe that no one has ever heard of it?  Dispite this site (which I really appreciate) I still fell totally alone. Undecided

Hey Strat - these things can really yank your whammy and short circuit your single coils can't they?  Wink Not to mention the damage they can do to your headstock that can make you feel like unbolting your neck.  Angry Gee willikers there are a lot of guitar twangers around here - does the knickel from the strings cause CH?  Cheesy

Although IMO it is easily the most dangerous drug prescribed for CH (if used in sustained, not tapered, high doses), it can actually knock an episode out for a small percentage of us. For others it will knock it down for a few days at the high dose, theoretically buying time for another slower ramping preventative to kick in. Possible risk (?): some of us feel our CH actually came on stronger, episode went longer, after a prednisone taper.

Yes 2 to 3 mg - that's what many of us including myself do with real success. RX says 2 days a week?? Never heard of that... 2 6 mg injections a DAY is conservative.My cycles started dramatically prolonging before imitrex was even frikkinn invented, so I don't know if it really prolongs 'em or not. I don't think it has prolonged mine any more than they already were.

If you could get a demand valve (not cheap) that stops the flow when not breathing that'd be SWEET I bet! Nice big M tanks are the way to go for us power users. Are you using a nonrebreather mask?

Using O2 as your first line abortive, imitrex on standby in case O2 doesn't work is a very effective, very proven approach. Going 25 LPM or more with non rebreather mask should get you some complete O2 aborts.

Some of us find that melatonin from 9 mg to 21 mg before bed can actually prevent the night hits.

Strictly relying on doctors for CH treatment is often a prescription that will keep you in a whole lot of PAIN. Doctors are WAY behind in many respects, and they are usually completely unaware of the benefits of high flow 100% O2.

There are lots of effective treatments documented at this site. Take matters into your own hands, learn as much as you can here and at clusterbusters.com, put it into practice, then tell this CH beast Jeebs said hello right before you bash him so hard with a swift uppercut knockout punch that his stupid horns go flinging right off.

BJBR...Thanks!  Nice to know someone's really out there.  I do not have the best mask, it's not a non-rebreather.  I taped up the side holes and just seal the mask to my face when I inhale and remove it to exhale...I try to exhale for twice as long as I inhale to maximize the vasoconstictive response to hyperventalation.  If the O2 defintely works I will have a demand valve overnighted to my house regardless of cost....I'd sell my car for relief.

By the way...although I get hit usually at night...it dosn't really matter what time of day or night...I get it full blown severe whenever I fall asleep, no exceptions for the past 4 days.

I had my first bout of CH pain in many years on 2/4/11.  I fell asleep on an airplane bound for Jamaca and woke up screaming out lould in my seat...they almost had to isolate me on the plane...they gave me an ice pack and it subsidied.  Then I got another bout that night and paced on the beach until the sun came up.  Then, PF for 6 days, nothing...but the day after I got back I awoke to the worst pain in my life...directly over my right eye.  Now I have had a severe headache for 4 straight days with intermittant episodes of cluster pain.  I can not believe that this is my life, really.  I just don't get why this is happening to me....why now...why me....why any of us?  If I didn't have a daughter I would think about the big "S".

O2 really does work - if I were you I think I'd be seriously looking at overnighting a demand valve too!

You hang in there strat - and there's more than one way to skin this beast BTW. Personally I like to skin it alive and send it screaming back to it's mommy.  Cheesy

Of course ya really oughta get the MRI and crap (if you haven't had one) and an official diagnosis from a headache specialist.

Then you can start looking into all manner of powerful beast killing activities, and you've come to the right place for advice and junk from those who've had some significant success. Smiley

Welcome to the board, the good news is you're not alone, and won't ever be again.

Are you working with a "headache specialist neurologist." GP's don't know squat about CH, and your garden variety neuro's don't know much more then that. From experience we've found people obtain the best results from a neuro, who specializes in headaches. The lingering pain you describe may be just what we call a "CH hangover", sometimes called shadows, or you could be a dual diagnosis case, CH and migrains, or ??...sadly we do have several on the board with multiple diagnosis. Makes the diagnosis and treatment much tougher.

Read this link when you get a chance. Just to make sure your maximizing the effectiveness of the 02. Most docs prescribe it at too low a flow rate, and like you, they give re breather masks, or worse, nasal canulas, which guarantee failure. I use a demand mask and swear by it. But they are pricey, upwards of $300.

You haven't mentioned a prevent med. A med you take daily, while in cycle, to reduce the frequency and intensity of your attacks. Verapamil is one of the more common prevents. Usually started WITH the prednisone taper as it takes up to 2 weeks to become an effective prevent. The prednisone will usually hold the beast at bay while the verapamil builds up. Lithium is what I use, at 1200 mg a day blocks 60-70% of my hits. Topomax works for some but is often called dopey max as it can make you a bit loopy.

This link will show you how to get 3 shots outta each of your auto injectors. Most can abort with just 2 mg.

Lastly, visit this site. They use "alternative" methods to bust their cycles, outside of mainstream medicine, but hard to argue with their success rate.

Thanks...I actually now do have the official diagnosis of "Cluster Headache with Chronic Daily Headache"

The neurologist says she has seen chronic headache sufferers actually develop cluster headaches after enough years.

All I want to do is learn to play serious guitar and instead I'm bouncing off the walls unable to think, let alone play.

G-Man...Thanks for the links.  Your link on the Imetrex is where I actually learned about using only 2 mg...interestingly, I didn't get the Imetrex injection medication I got a bag of siringes and 5 vials of sup. so it's really easy for me to just draw what I want.

Where can I get a demand valve locally (what type of company)?  The med supply that filled my O2 RX didn't even know what the hell I was talking about when I asked them about it!

I can't really offer any advice, but my husband's doctor also told him to limit the Imitrex injections to 2-3x a week. I don't know why. He also has started getting maybe a similar headache like you're referring to, but it's only been since he started the Imitrex a few days ago. (And now he's kind of afraid to stop the Imitrex since some readings have said ceasing it can cause more CHs...I don't know.)

Welcome aboard Stratocaster, as you can see there a few Musicians here, myself included. '85 Strat, candy red, '61 Gibson Melody Maker, '84 Gibson acoustic.

The mask you have is not very good for CH. That's why you're going thru so much O2 and not getting good aborts. Getting the best O2 setup you can, WILL give you much better results.

I was in Batch's High Flow O2 study and went from using 15lpm and a NON-REBREATHER mask, aborting 60% of my hits, to  the O2PTI mask and 25lpm+ and hyperventalating, raised this to 97% aborts in less than 5min.

At the end of my last cycle (July 2010) I bought a Demand Valve. If you buy one of these you will need a regulator with a DISS fitting. I used Flowtec for this and paid $167 for a reg that goes up to 60lpm with the DISS, about $20 for the DISS.

But, the O2PTI mask is nearly as good as the Demand and is made just for CH.
