Hi everyone. My name is Lynda (or Lou). My husband is a clusterhead:) We live in Rotorua, New Zealand.  My husband first experienced and was diagnosed 3 years ago by our GP (who has actually been researching CH for a long time) so thankfully she knew exactly what Haydn was going through straight away.  With his first attack we didn't look into anything and when they went it was like "well that's over" not realising the full extent of what he had.  Now he is going through his second cycle so I googled and here I am at this wonderful website. We have two small children and its been quite stressful trying to keep them quiet Smiley But to be able to read about others going through all this unbelievable pain helps me to understand Haydn and to keep calm with the kids.  Haydn is taking some sort of Steroid to try and prevent his headaches, he was on something else (amirol?) but it wasn't working. He is also taking Sumatriptan injections when he is being attacked (and they work within 5 mins). Since finding this website and trying different things that everyone suggests, he is able to get through his headaches without injecting himself (although he still does in the middle of the night).  I am about to log off and to go and buy him some Red bull for while he is at work:) He is in his second week of the cycle but he said last night that the pain was lessening, not sure if its the new drugs for preventing them or if he's not coming out the otherside of the cycle. But now that we have actually looked into this we are going to be much better prepared for next time. I just hope that its another 3 years and doesn't come any sooner. My heart goes out to all you ClusterHeads! And thank you so much for sharing so that others can find some comfort!

Sounds like you have found doc who knows the basics of treating Cluster. The steroid works quickly to stop attacks but it's used for only a few days. AT THE SAME TIME, he should have been given a preventive med which is used continuously (during periods when attacks are active). This med reduces the intensity and/or frequency of attacks. You continue to use the aggressive abortive med when an attack breaks thru.

ABSTRACT: Cluster headache (CH) is a primary headache disease characterized by recurrent short-lasting attacks (15 to 180 minutes) of excruciating unilateral periorbital pain accompanied by ipsilateral autonomic signs (lacrimation, nasal congestion, ptosis, miosis, lid edema, redness of the eye). It affects young adults, predominantly males. Prevalence is estimated at 0.5-1.0/1,000. CH has a circannual and circadian periodicity, attacks being clustered (hence the name) in bouts that can occur during specific months of the year. ALCOHOL IS THE ONLY DIETARY TRIGGER OF CH, STRONG ODORS (MAINLY SOLVENTS AND CIGARETTE SMOKE) AND NAPPING MAY ALSO TRIGGER CH ATTACKS. During bouts, attacks may happen at precise hours, especially during the night. During the attacks, patients tend to be restless. CH may be episodic or chronic, depending on the presence of remission periods. CH IS ASSOCIATED WITH TRIGEMINOVASCULAR ACTIVATION AND NEUROENDOCRINE AND VEGETATIVE DISTURBANCES, HOWEVER, THE PRECISE CAUSATIVE MECHANISMS REMAIN UNKNOWN. Involvement of the hypothalamus (a structure regulating endocrine function and sleep-wake rhythms) has been confirmed, explaining, at least in part, the cyclic aspects of CH. The disease is familial in about 10% of cases. Genetic factors play a role in CH susceptibility, and a causative role has been suggested for the hypocretin receptor gene. Diagnosis is clinical. Differential diagnoses include other primary headache diseases such as migraine, paroxysmal hemicrania and SUNCT syndrome. At present, there is no curative treatment. There are efficient treatments to shorten the painful attacks (acute treatments) and to reduce the number of daily attacks (prophylactic treatments). Acute treatment is based on subcutaneous administration of sumatriptan and high-flow oxygen. Verapamil, lithium, methysergide, prednisone, greater occipital nerve blocks and topiramate may be used for prophylaxis. In refractory cases, deep-brain stimulation of the hypothalamus and greater occipital nerve stimulators have been tried in experimental settings.THE DISEASE COURSE OVER A LIFETIME IS UNPREDICTABLE. Some patients have only one period of attacks, while in others the disease evolves from episodic to chronic form.

G'day Lynda, and a big welcome from over the ditch. We all know, of course, that none of this would have happened except for the AllBlacks but we're happy to be nice to you all the same.

You're real lucky in that there's a mad Kiwi here named Mike who's always ready to help others similarly affected and he's got all the local goss so can get things happening quickly for you.

It's also important you know that we reckon supporters like yourself are special people.

Welcome, Lou; and thank you for being there for your husband. Our supporters are vital to our well being. As you check out the site, please pay special attention to the "oxygen info" link. When coupled with Red Bull, or some other energy drink with at least 1000mg of taurine, it reduces abort times dramatically. Good luck and God bless. lance

While I didn't understand a single word of Aussie brians post......how the heck can you get that much of an accent into the written word???? Grin Grin....I must warn you that at some point he will insist you send him beer to avoid further headaches.....it's perfectly polite to avoid said requests! Wink

Bob's given you some great advice, I'll repeat what he said about the steroids as I'm concerned you might be in for an unpleasant suprise. Prednisone, at doses as low as 30 mg a day, will provide 100% releif from attacks for me. I do a 2 week prednisone taper at the beginning of my cycle, while I start on my prevent medication. For me that's lithium, Most prevents take about 2 weeks to really become effective, thus the 2 week prednisone taper to keep the beast at bay.

I tell you all that because the times I did prednisone by itslelf, when I came off the pred, the beast came back with a vengeance. Like the prednisone was only acting as a dam, letting a bunch of attacks build up! Sad

For a small percentage of people who suffer from CH, a short prednisone taper will actually abort the cycle. I'm hoping that will be the case for your husband. If it doesn't abort the cycle, make sure he has a sufficient supply of the injections in case beasty comes back hard.

And thanks for being such an awesome supporter. My wife found this site many years ago and saved my sanity. Our supporters are considered gold around here.

Hi Lou

First time I've seen anyone posting here from Roto-Vagas (which'll confuse a few people). Thanks for visiting for your husband. You will soon have learnt a lot more than most doctors know here and will be able to help to get him good, effective treatment for his CHs.


Dealing with CH you need a good preventive. I use verapamil with most people needing 360-480mg a day, although some go to double that dose. There are others like lithium too, which will prevent some of your CHs from happening.

Most preventives take a bit of time to build up, so it's common to use a prednisione taper (probably the steroid you mentioned), starting at about 60mg and tapering off to nothing over 10 days or so. This will work as a good preventive whilst the longer term preventive builds up. Just don't let him stay on it too long as prednisione isn't good to your body long term.

Once he gets a CH he wants to kill it as quick as possible. Imigran will do this, but oxygen is even better and it doesn't have any of the side effects of imgran. It isn't too easy to get this in NZ, however people I've spoken to here in NZ have got it. Most people here once they've got oxygen working well find they use the imgran (imitrext) injections a lot, lot less as it's about the same time to take effect and it's a lot less in the way of side effects too.

If he has nothing else around, try using a Red Bull or similar energy drink with taurine and caffeine. Just drink it right as a CH hits and you can normally reduce the intensity and duration of a CH. For a bad CH I'll combine it with the oxygen too.

Keep reading and keep asking questions... Hopefully his CH's will vanish quickly this time as spring is here and he'll want to enjoy the better weather.

As for Brian he is just overflowing with Culcha beyond normal belief.
