Hey Jason , I have coming tomorrow the unit that the store here sells . I had been using a face mask from amazon with the PSI almost to full .Jason Z wrote on Aug 21st, 2019 at 12:54pm:

I am not an expert at all but, it seems like O2 for 90 minutes with no effect is very strange. What pressure do you have your canister set to? 12psi seems to be the minimum. Also, are you using the face mask that covers the nose and mouth? A nasal cannula won't do the trick.

Hi Beast Mode and welcome

It sounds like you've an oxygen concentrator and are not using oxygen cylinders. The concentrators don't output pure oxygen and they only give out a low flow rate. You need 15lpm (or higher) as a flow rate and to be using a non-rebreather mask.

A 240mg / day verapamil dose is pretty low with most needing 360-480mg/day. I wrote a detail post about verapamil - Multimedia File Viewing and Clickable Links are available for Registered Members only!!  You need to Login or Register.

I'm confused as to why you've been given Metoclopramide as this is to help with stomach / nausea problems, which is more common in migraine. I'd ask your doctor more about why you've been given this.

An 8 hour CH is also pretty unusual too with the IHS (International Headache Society) having a standard definition of the diagnostic criteria for CH (Multimedia File Viewing and Clickable Links are available for Registered Members only!!  You need to Login or Register) which includes:

Severe or very severe unilateral orbital, supraorbital and/or temporal pain lasting 15-180 minutes


So 8 hours is significantly longer than the normal upper limit of 3 hours, although this doesn't automatically mean that it can't be a CH, more that most people have most of their CHs last 15-180 minutes.

It is also uncommon for CH to be treated using other the counter pain killers as for most people even the strongest narcotic pain killers don't really touch it.

Are you working with a headache specialist or just with your GP? We often find that GPs, even most neurologists simply don't have the skills or experience to properly deal with complex headache types like CH.

Hey Mike

Just working with my GP , my referral to see a Neurologist in Spokane was denied because I use a mild narcotic Tramadol for my lower back pain . Had fusion surgery for my lower back in 2016 and it helped some , but my golfing days are long over. I feel the Verapamil has been starting to help me , the CH are not as intense or last as long now . Have really upped my water intake also . Right now the best relief I get when the CH comes on is two take 2 OTC ES Migraine Relief capsules . The CH is gone until the next day , but I still get one CH a day . The Metoclopramide capsules have not helped at all . I plan to talk to the GP's nurse and update her on my condition . I did take the Cluster quiz on the web site here and I checked about 80% of the boxes for having CCH .

I'm totally confused why a referral to a neurologist was denied because you take a medication for your back, no matter what it is. People can often have one, two or more medical issues which are completely independent.

But then I'm not a medical doctor...

Please direct your GP / nurse to the IHS diagnostic criteria as I'm not totally sure that you may have the right diagnosis. This is based on other the counter pain killers making a difference. With most CHs they won't even scratch the surface of the pain.

There are a large number of headaches and some people are lucky / unlucky to have more than one type, which is what makes things complex to diagnose, hence our recommendation around a headache specialist.

Getting the diagnosis confirmed is the first step to getting the right treatments.

We've all been through the stages where you are, getting some answers, but not all. The average person with CH takes a few years to get the right diagnosis.

Yes ,  my CH are a head scratcher .Sometimes I get jaw pain on my right side before the CH starts up on the right side always . Never any pain at all on my left side of jaw or head . I always have a slight pressure like my head is in a vice most of the time . Between the meds and the 02 plus the Vitamins the intensity has come down on my CH's . Use to be a solid 7 or 8 ,
now they are in the 4 - 6 range . I usually get the headache as soon as I start moving around in the morning . But the 02 along with the OTC's takes care of them for the day .
