Looks good. You might want to check the spelling of the word "preventative" though...sorry, can't help it - have done a lot of proofreading in my career. Sure wish we could find a way to list doctor's names by area so other sufferers who are not having any luck with their own docs can benefit from those who do believe in Clusters. Don't see how this would work with this form though.
You might want to add a few blank lines at the bottom so sufferers could input remedies that have worked for them (like the post a while back about sticking your head in the freezer). You never know, we might just hit on something that could turn out to be universal!
