Seems like it should be fairly easy to weed out the wannabe's:

Now, if the patient reacts to the nitro, he/she is tatooed on the forearm (as in NAZI concentration camps) and taught the ultra-secret handshake of the damned.
Really though, I just hope it never gets to the point where we don't trust one another or feel that we have to prove to each other that we belong in this family.
