How long have you been taking the verapamil (this cluster)? It always, always always takes AT LEAST ten days for it to get into my system and give me any relief, so if you just started it recently, hang in there. I was chatting with a fellow the other night who apparently passes out regularly during a cluster, due to the severe pain. I guess your body can only handle so much, then it just shuts down.
As for waiting the in ER for four hours, I've been there, too. Makes you want to go postal on the idiots working there, who, with their VAST medical training should know that after the first 30 minutes to 1 hour, they might as well send your ass back home for all the good they can do you. Was it a military hospital, by any chance?
