FMLA rules are 12 weeks in a rolling year, starting when it becomes effective. You have to work 12 months in the previous year, Its for medical care for your immediate family, which also includes yourself, The 12 weeks can be taken a day at a time or any way that you need it. You would need a certification from the DR. to recieve the time off. Its a government form, so if your paper work is filled out properly, your company can not refuse the FLMA, it's the law.
