It probably is usually used in the sense of describing a cluster alone, however, I may have a clue for you as to what one really is -- although I hadn't heard the term applied to it. But my condition is a hybrid of a cluster and amigraine headache. The intensity, and many other things are relating to cluster while it is a bilateral, rather than a unilateral (never during a single headache, however) condition, as well as other migraine symptoms. So, the term may actually have an honest origin.
