Jack, I think this is a terrific idea; if we add this to the survey information, I think it would really flesh it out a little bit.

I'll use your format if that's ok:
First attack at 18 years of age - episodic for the last 13 years.
Cycle varies from 4-6 weeks long. Verapamil seems to extend the cycle. Middle 2-3 weeks most intense.
Sleep is also my principal trigger and will also get attacks durng the day during intense weeks.
Time of year has changed as I get older. Used to happen at any time of year; now is always fall with shadows in springtime.
First 10 years or so were unmedicated or wrongly medicated (Tylenol Sinus & Advil--yikes!!).
Primary meds currently: Verapamil ranging from 120 mg/day up to 540 mg/day when intense. Imitrix to abort; Vicodin for pain when needed and Compazine for nausea (caused by the Vicodin).

Otherwise, I'm in excellent health.

Also as I get older, the CHs are almost exclusively on left side, where it used to be either side.
