A1) From what I have read up to now, if O2 helps someone it will not lose its effectiveness over time. (You must be the world champ with your 1-2 mins.)

The most important thing about O2 is: Go to it as soon as possible when you feel an attack is coming; if you are already in full glow it is much less effective, if at all.
So, for those nightly attacks, train yourself to wake up at the first twinge (don't ask me how :)
Also, very helpful is a very long hose: This allows you to make your nightly trip to the bathroom after you hooked up to the O2, saving a precious minute ot two.
And of course, follow the usual recommendations for flow rate, mask type, etc.
