Welcome to our board (you are included in the "our"). I can't help with Q #1 but for me, triggers are. in order, MSG laced foods or snacks, heavy scents (perfume, solvents, pipe and cigar smoke, etc) and preservatives. I avoid all as much as I can when in a cycle but out of cycle only MSG bothers me with a cluster like HA. O2 works for me most of the time but when it doesn't, I take the beast for a walk. As far as cigarette smoking, I'd love to say yes, I quit and the CH went away but after 3 months of no smoking and loads of pain I started again (damn). Did the same with alcohol but it did make a difference in the intensity, the 10s went to maybe 7s. Good luck.
