You're absolutely correct that the "medinsurance" debate is much bigger than CHMB. Keep us updated on how your Insurance coverage changes if/when your Health Insurance Company goes from BC/BS, (non-profit) to Anthem, (for-profit). That should prove to be quite interesting indeed!
I can tell you this. Non-profit Health Insurance Companies are NOT allowed to give their employees bonuses. For-profit Health Insurance Companies ARE allowed to give their employees bonuses. This most certainly will have an affect on the cost that is passed on to you.
Personally, my doctor and I don't mind filling out a bunch of papers for BC/BS once a year or making a toll-free phone call.
P.S. I checked with my BC/BS source and she tells me that BC/BS of New York would have covered your therapy lamp under "durable medical equipment" coverage.
