Good questions - but some of them may be part of the problem

I noticed you referred to
"sinful lifestyle"

go back and reread the posts leading to that question
there is no reference to such a dynamic
you're bringing that to the discussion, after the fact, and then asking questions as though it had been part of the intitial question or premise

that's a major logic flaw AND basically renders the questions irrelevant - at least in the context of THIS discussion

AND, that may be a BIG part of the problem in trying to figure out a LOT of medical problems-
especially CH

in this case:

when any one or more of a handful of topics are mentioned,
many many many people IMMEDIATELY read the same old "God'll get for that" message in the topic - and apparently don't see that idea was never brought up, nor is it even vaguely relevant

once that happens, any chance of accurately looking at the factor is GONE, because of the tendency to bring in completely irrelevant issues
and apply them to the considerations
~~~~~~~~~~~~
then, there is the problem of trying to jump six steps at once -
sure - knowing WHY certain substances trigger attacks at certain times and not others is a good goal -
FIRST it takes clear accepted understanding that they DO act that way
you CAN'T work an complicated geometry or algebra problem backward from the solution -
you have to start where you are and take it bit by bit, using what you KNOW as a base for each advance
and if you aren't clearly sure of where you are or how you got there, it is necessary to back up, break the issue down into even simpler smaller chunks, and work through THEM until your path is based on solid, progressively supporting, understandings
and then - when you do get there, you're done
and not before
