Some insurance companies don't automatically OK it because the doctor says it's needed. They need to be proven that from your doctor. Call the insurance company and talk to a case manager specifically about O2 and clusters, concerning his policy. If it's not OK'd, have your doctor send them whatever they need to get it OK'd.
