I got to thinking you may be wonering yourself..."what tools?"..or "what specifics?"
Here are some things you should include along with your template letter in your post:
Who is your medical carrier?
Is your carrier through your workplace, your personal coverage, an HMO, or government?
Who is your doctor?
What has he done to help?
Who diagnosed chronic CH?
Who are "they" that 'do not recognize chronic CH'?
How long have you been using injections?
Who paid for it up until now?
Keep asking yourself as many questions as you can think of, they are all relevant.
This may sound unpassionate, but it is not. I truly would like you to win.
