From the few people at my work(that experienced the day I had a 10+ in the bathroom). A few of them have told my "it's ok I have migraines too." Even my boss thinks I had a migraine(never tried to explain to her anything different).
As far as my close friends they accept my explanation but only my fiance, my parents, & my sisters really know the pain because they've seen me in ch form too many times.
It doesn't bother me that the general public doesn't understand...I don't expect them to.
