I usually start with the ice right on the pain site. But as the HA progresses, it hurts too much to keep it there. The ice pack gradually works it way back. My thought is that the cold will help constrict the blood vessels.
I think the exercise at the onset of the HA acts as a poor mans O2. The huffing & puffing oxegenates the blood.
