According to the second edition of Lawrence Robbins's "Management of Headache...",
some CPH patients may require as much as 250mg or more of indomethacin (Indocin) per day (the most effective
dosing seems to be 25mg or 50 mg taken throughout the day, rather than the more convenient 75mg).
However, he also notes that "if indomethacin does not work, the diagnosis of
CPH is in doubt." Nevertheless, a few patients respond to ergots, NSAIDs, tricyclics, or verapamil.
Robbins adds that "if extremely high doses of medication are required, we think
about secondary causes," including tumors, collagen vascular disease, cerebrovascular disease,
aneurysms, arteriovenous malformations. (Yikes!)
