I'm really so sorry you're hurting so much, but at least you've found another, I hope temporary home here where we all understand.
I'm really not familiar with the medications you're taking, but have you tried oxygen, for openers. It's a quite fast abortive. Has your doctor, your neurologist I hope, suggested verapamil, or a prednisone taper? Then there is topamax, which has worked wonders for many of us here.
I mention these treatments only because they have been quite effective for a lot us, and are a sort of front line attack on the headaches. If you haven't tried them, then speak to your neurologist, and if he or she is not familiar with them, then I would get another doctor who is, and fast.
