I am somewhat disappointed in my masterwork. When I went through it to answer the questions myself I wondered in some places what I was meant to say. Should we put a N/A if a comment question doesn't apply so I will know it was not just overlooked by accident?
It took me about 26 minutes to answer my own questions. I was not thorough and did not give it much thought or make very complete comments. After 3 weeks of toil on this I am totally sick of it!!!
Anyway, some of you already sent me suggestions about style and some new questions to add. I incorporated those into this latest version.
Then I sent several inquiries to my server who seems not to be able to understand my questions - which may mean my questions are stupid.
Anyway, I had to rearrange pages to make the "send" button work - I am assuming it does work since the "reset" button next to it works - that may not be a valid comparison, but previously neither worked.
I still have a few questions and revisions to make.
If you wish to PREVIEW the questionnaire in order to critique or offer suggestions, that is fine. I can still use your input. But please do not hit the "send" buttons yet. Please wait until all the revisions have been made so that the questionnaire will be the same for everyone.
