Some times I have had some luck spending some time on the phone with the Customer service/complaint dept. Write down who you talked to date. Then follow up with a letter and one from the Dr.. I have had some luck by this. We have had some luck calling the State Ins. Commission. It's a pain and doesn't always work. Maybe worth a shot.
