(if different places, where was first longterm residency closest to birth ?)
3. what is the predominant GENERAL region-of-origin roots of EACH of your biological parents, back several generations
(eg: Scandinavia, northern Europe, southern Africa, etc.) and SPECIFY for mother/father - (the point of this is not ethnic type from a culture aspect, but LOCATION of origin )
