Shari, First of all, let me say, he's one lucky man to have a wife that cares enough to try to get him help.
Has he had an MRI? If not get one immediately. These $%^& headaches can hit at any time - day or night. Does the Imitrex give him a few hours relief? If so use it! If you have any doubts about your dr. FIND A NEW ONE! That seems to be a big problem - finding the "right" doctor, one who KNOWS something about CH and is willing to "experiment" until the right med is found.
We all react differenly so you might try heat or cold (which ever helps) while he's on O2. The O2 needs to be on at least 8 Liters for about 10-20 minutes at a time and you need to use a face mask (NOT the nose thing).
