When I first started getting the headaches, I went to various types of doctors; not a neurologist. My dentist swore up and down that TMJ caused the headaches; denied that it could be anything else. I had the mouthpiece made and wore it for 6-8 months thinking that was it (what I didn't realize was my cluster period was over when I started wearing the mouthpiece). When the next cluster period came, the mouthpiece gave no relief (actually, I snapped it in half during a CH). Then I finally went to a neurologist. The first words out of his mouth after my description of the pain(s) was "cluster headaches." Let me add that it was 6 years of 2 cycles/year before I decided to see that neurologist. I never had a doctor tell me he/she thought it was the "M" headache.
