Every single Blue Cross Blue Shield in the world is bound by their OWN particular policies and STATE regulations. The BC/BS that Ted has posted is for Massachusettes.
Example: The BC/BS in Tenessee may not have a clue about Cluster Headaches. It's up to you and your doctor to educate the Medical Director at your own specific Health Insurance Company.
