the apnea is related to the smoking.
BTW, the Goadsby book has one study which refers to the Kudrow study. In a broad "Case-control study on the epidemiology of cluster headache" (Granella and ICECH), the only statistically significant correlations found among Ch'rs vs. paired controls are 1)higher level of smoking (both incidence and # of cigs per day), 2)previous head trauma (30.8% vs 15.8%) and 3)family history of headaches (56.7% vs. 35%). The authors note that the head trauma and family history stats could be subject to "recall bias" and therefore are less reliable than the smoking.
Of the 120 CH'rs and 120 controls in the study, 40 CH (33.3%) and 73 (60.8%) had never smoked.
