I cannot tell you who establishes the code, I would guess that it is a joint project with the Medical community and the insurance companies. You have peaked my curiosity and I will find out.
As you found, not every condition is coded. BUT, that does not preclude a Doctor from taking the additional step of writing a synopsis to clarify the code. That should be done with CH because of the variations to the treatment, as with other medical conditions that do not have a specific code.
