Blue Cross/Blue Shield is kind of a separate case. While the name is consistent, it actually consists of many, many "affiliate" companies. In other words, BC/BS of Florida is pretty much totally independent of, and thus has different policies, than BC/BS of Wisconsin (for example).
In some larger states, there are actually several BC/BS companies.
Check with Drummer for more info - his wife works with BC/BS.
BTW, here in Florida, BC/BS limits me to 2 Imitrex injection kits/month or 9 tabs.
