I have taken high doses of both types, (extended and immediate) and I have found that the immediate relief pills are much more effective. My current dose is 720 mg/day, and I take 240mg at 7:00 am, 240mg at 6:30 pm and 240mg at 11:00 pm.
So far, it has knocked out the cycle completely, shadows and all.
