two-part questionnaire responses. You see, we only use ID codes, and I do not have the names or email addresses of anyone who has responded.
Maybe if you opened the Q to All Sufferers and read through the questions you would get a feeling for how familiar it is to you. Also take a look at the second form - either chronic or episodic. Maybe this will jog your memory.
Another hint, this questionnaire was first offered around February 13 of this year.
And lastly, do you remember putting in an ID Code of some sort. I asked for a 5-character one of your choice - but it can actually be 4 or 6 characters. The script does not care. I just chose 5 at random.
