Danny, After trying ice, which actually seemed to worsen the situation in my case, I also found heat and the heating pad would offer a margin of relief. When it's effectiveness began to wane (as they all seem to do)I started using the shower. Sitting in the tub and using the hand held shower, I run water, as hot as I can stand, on the affected side of my head. The heat seems to help as well as the activity of slowly moving the flow around to each of the pain centers ie, forehead, temple, crown, ear, etc. All of this gives me something to do (rather than pacing or banging my head) and helps somewhat to relieve the pain while I wait for the current meds to take effect. It also relaxes me as much as possible so I may quickly return to sleep. Hope this helps. One word of caution... in colder climates it is advisable to run a little hot water on the bottom of the tub before you bring it in contact with delicate body parts. Fends off the initial shock.
