If they have any doubts a glimpse at the very specific CH questions will alert them to their mistake. If a prankster submits the forms and writes in a lot of migraine symptoms, Paul can simply discard that form. And there is the margin of error factor to help us here.
Frankly, I doubt there would be many with the patience to plow through a questionnaire that obviously has no bearing on migraine pain. Have you studied the forms? Take a look. These forms would put a non-clusterhead to sleep.
You will have to think up a better excuse than that to get my attention.
