the nurse I talked to didn't seem to have a clue what the insurance co. wanted in letter form...that's why I had to make an appointment w/ doc, so we could do letter together. I'm going to call the insurance co Monday and ask them exactly what they want on the letter too.
Thanks for the suggestions that should be in the letter, they should help alot re: a permanent medication.
