OK to reduce dose when chronic under control?

If someone is chronic and finds a medication that seems to have got things under control, should they just keep taking the medication at the same dose or should they try to reduce it after a while? Also, if the current dose seems to stop working and they need to increase it to improve things, should they then try to decrease it again after a while? Then they could try to keep the drug at the lowest level and have a lower base in case they need to increase it again in future. Not planning to reduce at the moment, but for future reference it would be useful to know people's experiences if they are chronic. Thanks for any advice!
