Definitely an "art" film and very black in mood. The hero is clearly a clusterhead from his reactions to the pain, and the catalogue of drugs he has used sounds very like a trawl through any cluster site search engine.
The only pity is that it is never referred to by name, and the director's commentary - available on the DVD version - refers to them as migraines repeatedly.
If anyone has any way of contacting the producer/director, it would be nice to find out whom they were modelling the hero's headaches on - there's clearly someone there with inside knowledge. The scene with the power drill is particularly vivid.....
