According to several sources (sorry, can't remember them all) that I have checked, it is not so much that smoking CAUSES CHs.
Rather, it is more like whatever defect in a clusterhead's body makes her/him prone to clusters also makes him more prone to nicotine addiction than average. It also makes him more prone to be a heavy social drinker (though not necessarily alcoholic) than average, although less so than nicotine addiction.
