The hypothalmus is situated too deep within the brain to be affected by blunt force injury. An alien would have to get in there with an eggbeater to affect it physically.
Clusters are caused by a deformity of the hypothalmus. Sometimes it's latent and doesn't present until later in life, but it's there, ticking like a time bomb. That's it. That's all.
