I contacted my HMO (Health Net) last year and started this proceedure so I would be set when my next cluster started. I figured that once it started I wouldn't feel much like going to battle over this.
The response from Health Net was that it was the physicians decision. If they determined it was necessary then the HMO would cover it. I would not be limited to 6 doses/month.
Naturally, I saved all of this correspondence knowing I will need to use it when the time comes.
