A couple of things you said caught my eye. They may be significant, or they may just be the way you worded things.

You said the pain is "over his right eye when he wakes up in the morning". You also said that "The pain always starts in the same spot over the right eye and always hurts more on the right side of his forehead"

I'm picking up on the "over his right eye", "starts in the same spot" and "hurts more on the right side". Does this mean the pain moves or spreads? If it hurts more on the right side, does it also hurt on the left side? Is the pain OVER his eye, or in his eye?

Combined with the duration, these things make his attacks sound more like migraine, assuming that potential causative precipitators have been ruled out (like tumor, etc). Either way, he needs to see a neurologist and get checked out completely.
