In the OUCH Liason committee we are trying to tackle just these sort of things one at a time.
"Preparing for the Doctor" is yet another form we hope to make available for printout at the OUCH site very soon. I'll request we take a look at preparing such a template chart either as a part of this form, or possibly as a side "tool".
In the meantime, do you have Excel? If so, you can e-mail me and I may be able to help you set something up. If you don't have Excel, I think we can still come up with something in another document program.
