albeit a sad welcome. I am sorry for Tim's pain as well as yours. Tim is very lucky though, supporters like you are few and far between.
As for your questions:
Jerks or tremors: Not me, but that is just my personal experience...the trick about this beast is that he affects each of us differently.
Gamma knife: There has been a lot of discussion here about that. I would suggest that you do a search of the archived posts (there is a search button on the top right hand corner of this page) for gamma knife and I am sure you will find many, many posts about it.
Work: Not during a headache, and sometimes not even in between them, as I am too wiped out to function.
About the dr's: There is another website you should check out too, I put the link below. It is the OUCH website (organization for the understanding of cluster headaches) and they have a list of docs broken down by area, that have been recommended by other sufferers. Hopefully, you can find a good dr close to you.
There are also lots of other options...please check out the links to the left....has Tim tried oxygen? Let us know what meds he has tried as well and we will try to come up with some others that have been helpful for us.
