We have an information form for employers of Cluster Headache suffering employees. This will soon be added to the OUCH website for printing in either Word or PDF formats.
If you have Microsoft Word, I can send anyone a copy at anytime.
The form is designed to best help employers understand just what their employee is dealing with. It includes suggestions for interaction between employer/employee to explore all options to ensure the employee does not lose their job.
It includes a description of CH intended to help employers understand CH, and few other areas that we hope will help the CH sufferer in their workplace. Most of all to help them keep their jobs! There are spaces you can fill in yourself (or better yet your doctor) pertaining to your specific CH profile.
If you'd like a copy, I'd be happy to send anyone one right away. But at this point I only have it available in the Word format. Soon, you will find it at the OUCH website under "Cluster Resources" ... "Dealing with Employers".
