The OUCH Liaison Committee is working to put together a Children's Diary. This project is intended to help you help your child. We plan to have an introduction page with information on how to use the diary and other basic information. A form for your child's school faculty and administration. A chart to monitor your child's daily routine's (times of attacks, severity, medications, results, etc.). And finally some information to help assist you in dealing with your doctors.
If you have any requests or concerns about anything that might also help you help your child that could be added, please contact me.
Or, we could use your address and a little information and send you copies for evaluation. We hope to help you and your child deal with this as much as we possibly can.
