The MS Word format was fine for me and I think most people are going to be able to use that quite easily. PDF files are more portable to differing OS's I think, but hey, most run PC's with the Windows OS anyway.
I read the form and it looks pretty darn good to me. Provides basic information about the condition while (If your doc fills it out) also lending authority and weight to it. Also provides directions to sites for more information. Good idea. For what my opinion is worth, it looks great.
I saved a copy of it to my Doc's folder and I will have to try it the next time.
I would be curious to find out if anyone else has had success utilizing it. It looks like it is worth the effort to me.
