another cause is more indirect due to fires associated with O2 tanks, or not properly supported causing the valves to break off where the tank will shoot off like a bullet.
Don't be skeptical until you breathe from a contaminated tank especially during a mean CH. I was using tanks for three years before I got one and from a medical supply house.
Doesn't help when you hear a voice over the phone in the background, "another contaminated tank!"
