I'm sure you did, but JUST in case you didn't... Recall the neurologist's office and say "I'm in the middle of a cluster and running out of options for treatment. I need to see him YESTERDAY." Some neuros actually have "magic words" with their staff that can bump you up in the appointment queue... Worth a shot if you didn't try it already... Another one I've used is to actually physically go to the office and sit, saying "I realize he's busy, but I don't think you realize how bad this is... I have a referral from my PCP, insurance covers it, and I'm not leaving until I have an appointment."

This sounds drastic and it surely IS annoying to the staff, but if you can spare the time it stands a fair chance of working.
